Uninstall a GitHub App installation

Uninstall the deployment GitHub App from this account (AgentConnect organization owners only), then mark its local provenance revoked and re-converge the organization’s GitHub hook rules. The provenance row is retained so a later reinstall can self-heal existing agents.
